(2)单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动；
(3)本项目不接受联合体形式。

(c)Specific qualification requirements for this program: (1) Not included in the list of dishonest persons executed by "Credit China" (www.creditchina.gov.cn), the list of tax violations and the list of serious violations of government procurement credit; (2) Suppliers with the same person in charge or directly controlled and managed by different suppliers are not allowed to participate in the same government procurement activities; (3) This project does not accept consortiums.

1、法人依法设立的分支机构以自己的名义参与磋商时，应提供依法登记的相关证明材料和由法人出具的授权其分支机构在其经营范围内参加政府采购活动并承担全部民事责任的书面声明。法人与其分支机构不得同时参与同一项目的采购活动。
2、根据上海市财政局规定，本项目招投标相关活动在上海市政府采购云平台（网址：http:****）电子招投标系统进行。供应商应根据《上海市电子政府采购管理暂行办法》等有关规定和要求执行。供应商在政府采购云平台的有关操作方法可以参照政府采购云平台中的专栏有关内容和操作要求办理。 供应商应在响应文件截止时间前尽早加密上传响应文件，电话通知项目负责人进行签收，并及时查看政府采购云平台上的签收情况，打印签收回执，以免因临近响应文件截止时间上传造成无法在开标前完成签收的情形。未签收的响应文件视为投标未完成。

1.
